- Prepare the narrative for the nomination. Click here to download a sample narrative. [PDF]
- Select the appropriate template below.
- Compose your narrative and save.
- Prepare a citation of 250 words or fewer. This citation may be used in the Honors Program if the nominee is selected to receive the award. click here to download a sample citation. [PDF]
- Complete the appropriate nomination form from the list below.
- Upload narrative, citation and any other required supplemental info and submit form electronically.
