Community of Interest Webinar Request Form
Webinars should be discussed with your National Staff Liaison prior to filling out this form. Please allow 30 days from the time you fill out the form to the date of the webinar. This allows the staff time to market your webinar, build registration, build the platform, and ensure that you have an audience! Missing or incomplete information could result in a delay or cancellation of your webinar date.
If your webinar is PDH worthy, you must fill out a PDH form (leaving participant name blank) to upload in the below form.
