SAME Foundation Program Impact Report Submission Form

This form is used to collect standardized data, testimonials, and other information to share with donors and members. The quality of impact and quantity of people served will support future programmatic requests, build trust in donors and members, and demonstrate SAME and the SAME Foundation’s commitment to the Strategic Goals within the 2030 Strategic Plan. Reports are due at the conclusion of a program to be included in the August publishing for the previous year. For example, if a program is a year-long program ending on Dec. 31, 2035, the report is due before April 1, 2036 to be included in the 2035 Impact Report being published in August, 2036.
